Arizona Licensed Telephonic Case Manager RN

Job Summary

A company is looking for a Telephonic Case Manager RN - Remote AZ.

Key Responsibilities

• Make outbound calls to assess members' current health status
• Identify gaps or barriers in treatment plans and provide patient education
• Coordinate care for members and make referrals to outside sources

Required Qualifications

• Current unrestricted Multiple state licensure - Compact License
• Ability to obtain CCM certification within 18 months of employment or currently a Certified Case Manager (CCM)
• 3+ years of experience as a Registered Nurse in a hospital setting or as a telephonic Case Manager
• Ability to navigate a Windows environment and utilize Microsoft Word
• Private home office and access to high-speed Internet
