A company is looking for a Care Coordinator to support patient care and engagement in a tele-mental health setting.
Key Responsibilities
- Utilize software and local resources to provide necessary information
- Follow up on no-show appointments and assist with rescheduling
- Support patient engagement through phone calls and chat messages
Qualifications
- Background in behavioral health, such as a BS in psychology or social work, or equivalent experience
- Comfortable working in a fast-paced, changing environment
- Self-starter with the ability to work as part of a team
- Experience or comfort with crisis management
- Bilingual in Spanish/Chinese is a plus but not required
