Registered Nurse Clinical Intake Coordinator

Job Summary

A company is looking for a Full-Time Home Health Clinical Intake Registered Nurse/RN Coordinator (Remote).

Key Responsibilities

• Gather necessary information from referral sources and evaluate referrals for client appropriateness for home health services
• Promote agency services and function as a clinical resource and customer service representative
• Receive and document medical orders, processing referrals accurately and timely

Required Qualifications

• 5+ years of Registered Nurse/RN experience
• 2+ years of home health RN experience
• 1-3 years of experience in customer service, homecare intake, case management, discharge planning, or admissions preferred
• Graduate of an approved Nursing education program
• Current Registered Nurse licensure in the state of practice
