Key Responsibilities
- Build and maintain relationships with health systems, clinics, and community organizations
- Develop and implement community-level initiatives for diabetes prevention and chronic disease management
- Monitor program performance and support resource development for ADA-funded initiatives
Required Qualifications
- Bachelor's degree in public health, nursing, pharmacy, health administration, or related field (Master's preferred)
- Minimum of 5 years of relevant experience in healthcare or community health settings
- Strong project management skills with experience in leading multi-stakeholder initiatives
- Established relationships with local clinical institutions and academic centers
- Ability to work independently and collaboratively in fast-paced environments
