Registered Nurse Care Manager

Job Summary

A company is looking for a Care Manager (RN).

Key Responsibilities

• Evaluate member needs and develop personalized care plans
• Coordinate care management interventions and facilitate access to services
• Provide ongoing monitoring and education to members and their families

Required Qualifications

• Degree from an Accredited School of Nursing or Bachelor's degree in Nursing
• 2 - 4 years of related experience
• RN - Registered Nurse - State Licensure and/or Compact State Licensure required
• Experience in OB and Physical Health (ICU, ER, Med-Surg, Ortho, etc.) preferred
• Must reside in Georgia or North Carolina
