Virginia Licensed Community Health Worker

Job Summary

A company is looking for a Community Health Worker to act as a liaison with Medicaid members and facilitate access to care.

Key Responsibilities:

• Complete new member processes, including welcome calls and health screenings
• Create positive relationships with members to enhance engagement with their primary care
• Address social determinants of health and support transitions of care

Required Qualifications:

• High School Diploma / GED (or higher)
• 1+ years of experience/knowledge of community resources and culture
• 1+ years of clinical experience or in a clinical role
• Intermediate knowledge of Microsoft Office applications
• Must reside in Virginia and have access to high-speed internet
