Job Summary

A company is looking for a Care Navigator (Virtual Healthcare).

Key Responsibilities

• Guide dyads through the dementia care journey by providing support, strategies, education, and resources
• Conduct comprehensive assessments to understand dyad needs and collaborate to develop personalized care plans
• Maintain communication between dyads, care teams, and healthcare professionals, while identifying and connecting them to community resources

Required Qualifications

• Associate's or Bachelor's degree required; relevant professional experience may be considered in lieu of a formal degree
• Minimum of 2 years of professional experience working with the geriatric population
• Prior experience with dementia or cognitive impairments preferred
• Ability to work in a virtual environment with a private and quiet workspace
• Strong organizational skills and proficiency in maintaining records and utilizing relevant software (EHR preferred)
