California Licensed Care Manager

Job Summary

A company is looking for a Care Manager Transition of Care.

Key Responsibilities

• Evaluate member needs through post-discharge assessments and medication reconciliation
• Develop care/service plans in collaboration with healthcare teams to support member transitions
• Facilitate communication and services between members, providers, and support resources

Required Qualifications

• Master's degree in Behavioral Health, Social Work, or a degree from an accredited school of nursing
• 2 - 4 years of related experience
• Licensure as LISW, LCSW, LMSW, LMFT, LMHC, LPC, or RN
• California residency is required
• Bilingual capabilities, preferably in Mandarin or Cantonese, are a plus
