Job Summary

A company is looking for a Care Connections Representative.

Key Responsibilities

• Conduct outbound calls to schedule appointments with Nurse Practitioners for preventive care services
• Meet call center metrics while providing high-quality customer service and removing barriers for members
• Document interactions in electronic medical records and participate in special projects as assigned

Required Qualifications

• High School Diploma or equivalency
• Working knowledge of Microsoft Office or comparable software
• Bilingual (English/Spanish) is preferred
• Customer service or call center experience in healthcare is preferred
• Associate's degree or equivalent combination of education and experience is preferred
