Care Guide Representative

Job Summary

A company is looking for a Care Guide Representative to facilitate patient care and support health goals.

Key Responsibilities

• Acts as the central facilitator to patients, coordinating care and communication
• Identifies and resolves barriers to patient health care goals
• Develops patient-driven goals and supports adherence to care plans

Required Qualifications

• Must be 18 years of age with relevant education and/or experience
• 0 to 2 years of experience in customer service, health care, or community-based roles
• High school diploma or GED preferred
• Bachelor's degree in health care is a plus
