Job Summary

A company is looking for a Health Coach - Remote.

Key Responsibilities

• Engage with participants through telephonic calls to inform and enroll them in health programs
• Provide individualized coaching to encourage behavior change and improve health outcomes
• Coordinate services with a multi-disciplinary care team and track member progress

Required Qualifications

• Bachelor's degree in a health-related field required
• 1-2 years of related experience required, preferably in customer service
• Proficiency in computer data entry and Microsoft Office
• Ability to coordinate telephonic communication while navigating software platforms
• Hard-wired internet connection is necessary as WIFI is not permitted
