Licensed Bereavement Coordinator

Job Summary

A company is looking for a Regional HSP Bereavement Coordinator.

Key Responsibilities

• Meet with care center leadership to assess and develop bereavement services for patients and families
• Conduct post-death bereavement risk assessments and create individualized bereavement care plans
• Provide community outreach and support to individuals grieving, including referrals to local services

Required Qualifications

• Bachelor's degree in social work, psychology, counseling, or a related field
• One year of experience in family and/or individual counseling with training in grief and bereavement
• Knowledge of grief and bereavement counseling and the needs of terminally ill patients and their caregivers
• Familiarity with general computer use and online HR services
• Self-management skills for online training and services
