Licensed Nurse Practitioner

Job Summary

A company is looking for a Nurse Practitioner Partner.

Key Responsibilities

• Conduct 5-10 hours of telehealth visits, providing consultations and prescribing medications as needed
• Collaborate with Community Health Workers (CHWs) to assess patient needs and eligibility for services
• Maintain accurate patient records and provide clinical oversight to CHWs

Required Qualifications

• Board certification as a Nurse Practitioner
• Minimum of 2 years of consecutive experience as a Nurse Practitioner
• Commitment to health equity and addressing social health needs
• Ability to operate independently with strong diagnostic skills
• Experience in telemedicine and proficiency with various digital tools
