Job Summary

A company is looking for a Physical Therapist - Outpatient at Home (New Grad Mentor Program).

Key Responsibilities

• Evaluate, assess progress, and provide a plan of care and exercise prescription for patient caseload
• Ensure a world-class level of clinical care and patient experience
• Document all treatments in the EMR and communicate with patients and/or providers as needed

Required Qualifications

• Current Licensed Physical Therapist in New York State
• Completed state regulated CEU requirement in NYS
• BLS certification
• Commit to a minimum of 4 visits/week
