New York State Case Planner

Job Summary

A company is looking for a Case Planner I (SBC) to join their Preventive Services Program.

Key Responsibilities:

• Provide case management and supportive services to families at risk of foster care involvement
• Conduct assessments, make referrals, and monitor client progress
• Participate in interdisciplinary team meetings with clinical staff

Qualifications:

• Bachelor's degree in Social Work, Psychology, Sociology, or a related field
• Knowledge of child welfare practices and risk factors for abuse/neglect
• Ability to work effectively with culturally and professionally diverse teams
• Spanish fluency highly preferred
