A company is looking for a Program Coordinator to support high acuity patients in their healthcare journey.
Key Responsibilities
- Process referrals and coordinate enrollment into high-risk programs based on established criteria
- Lead the enrollment process for home visit programs, including scheduling and administrative tasks
- Provide administrative support for planning and confirming home visits, and manage incoming calls from patients
Required Qualifications
- At least 2 years of experience in a high volume call center or sales position
- Experience in a healthcare setting is preferred
- Strong organizational skills and ability to manage multiple priorities
- Ability to adapt to change while maintaining productivity
- Proficiency in computer technology and software systems
