Program Coordinator

Job Summary

A company is looking for a Program Coordinator to support high acuity patients in their healthcare journey.

Key Responsibilities

• Process referrals and coordinate enrollment into high-risk programs based on established criteria
• Lead the enrollment process for home visit programs, including scheduling and administrative tasks
• Provide administrative support for planning and confirming home visits, and manage incoming calls from patients

Required Qualifications

• At least 2 years of experience in a high volume call center or sales position
• Experience in a healthcare setting is preferred
• Strong organizational skills and ability to manage multiple priorities
• Ability to adapt to change while maintaining productivity
• Proficiency in computer technology and software systems
