Program Manager - Population Health

Job Summary

A company is looking for a Program Manager - Population Health Management.

Key Responsibilities

• Manage the implementation and administration of population health programs
• Coordinate cross-functional resources and third parties/vendors for program execution
• Track program performance and report to management as needed

Required Qualifications

• Bachelor's degree in healthcare administration, nursing, public health, or a related area
• 2-4 years of experience in healthcare administration or a related field
• Proven experience in program management
• Familiarity with tools such as Asana, Microsoft Teams, and advanced Excel
• Clinical license (RN, LCPC, LCSW, etc.) is a plus
