Bilingual Call Center Representative

Job Summary

A company is looking for a Call Center Representative - Bilingual (English/Spanish).

Key Responsibilities

• Assist members with completing assessments and recommend service plans based on their needs
• Make outbound calls to follow up with members and coordinate services
• Support members in accessing community resources such as homecare, meals, and transportation

Required Qualifications

• High School Diploma preferred
• Experience in Public/Community Health or related fields preferred
• Experience in social services, care coordination, or healthcare navigation
• Must be 18 years of age or older
• Computer proficiency and knowledge of clerical functions
