A company is looking for a Care Coordination Specialist.
Key Responsibilities
- Conduct assessments to identify individual member care needs and preferences for high-quality care coordination
- Collaborate with members, health plans, providers, and community organizations to facilitate access to dental care
- Monitor members' care and maintain records of decisions, actions, and outcomes related to care coordination
Required Qualifications, Training, and Education
- Minimum of three years of experience in a Care Coordination position within healthcare or insurance, with Medicaid and/or Medicare knowledge preferred
- Medi-Cal Dental health experience and knowledge of dental treatment authorization processes preferred
- Experience supporting individuals with disabilities, chronic illnesses, or homelessness, with an understanding of social determinants of health
- Familiarity with care coordination methodology and ability to work independently and in a team
- High school diploma required; clinical or administrative dental or medical experience preferred
