Job Summary

A company is looking for a LVN, Case Manager - Remote CA.

Key Responsibilities:

• Ensure continuity of care in inpatient and outpatient settings
• Conduct reviews of patient care and coordinate treatment plans
• Act as a liaison for patients and families regarding care and discharge planning

Required Qualifications:

• Graduation from an accredited Licensed Vocational Nurse program
• Current LVN license in California
• 1+ years of recent clinical experience as an LVN/LPN
