Cardiac Care Health Advocate

Job Summary

A company is looking for a Health Advocate to join their virtual telehealth-based cardiac clinic.

Key Responsibilities

• Conduct biweekly outreach calls to patients, providing support and coaching on lifestyle modifications
• Monitor and encourage adherence to prescribed treatment plans and escalate clinical concerns to the team nurse
• Guide patients on the use of remote patient monitoring devices and maintain accurate documentation of patient interactions

Required Qualifications

• Minimum of 2 years of experience as a medical assistant, preferably in cardiac care
• Medical Assistant certification is preferred
• Knowledge of cardiac conditions, treatments, and common medications
• Comfortable with virtual telehealth platforms and basic computer skills
• Ability to work collaboratively with a multidisciplinary healthcare team
