A company is looking for a Care Guide Representative.
Key Responsibilities
- Acts as the central facilitator to patients and supports them through all stages of care
- Identifies and resolves barriers to patient care and ensures connections to necessary resources
- Develops patient-driven goals and utilizes behavior change techniques to empower patient participation in their health
Required Qualifications
- Must be 18 years of age with relevant education and/or experience
- 0 to 2 years of experience in customer service, health care, or community-based role
- High school diploma or GED preferred
- Bachelor's degree in health care is a plus
