Job Summary

A company is looking for a Care Management Associate to support the coordination of medical services and referrals for members.

Key Responsibilities

• Support quality effectiveness of healthcare services and coordinate member care transitions
• Create authorizations in QNXT and screen incoming forms to identify needed medical services
• Facilitate healthcare outcomes by providing care coordination, education, and follow-up services

Required Qualifications

• High school diploma or GED required
• 1-2+ years of call center experience
• Fingerprinting required for this position
• Remote work with potential future onsite requirements
• No prior CVS/Aetna work experience required
