Job Summary
A company is looking for a Care Management Support Coordinator II.
Key Responsibilities
- Provides outreach to members and supports care plan next steps and resource connections
- Acts as front-line support for member and provider inquiries, explaining care plan procedures
- Documents and maintains non-clinical member records in compliance with state and regulatory requirements
Required Qualifications
- High School diploma or GED required
- 1 - 2 years of related experience preferred
- Knowledge of health plan activities and community resources is beneficial
- Ability to connect members to health plan and community resources
- Familiarity with Social Determinants of Health (SDOH) needs is advantageous
