Job Summary

A company is looking for a Care Management Support Coordinator II.

Key Responsibilities
  • Provides outreach to members and supports care plan next steps and resource connections
  • Acts as front-line support for member and provider inquiries, explaining care plan procedures
  • Documents and maintains non-clinical member records in compliance with state and regulatory requirements
Required Qualifications
  • High School diploma or GED required
  • 1 - 2 years of related experience preferred
  • Knowledge of health plan activities and community resources is beneficial
  • Ability to connect members to health plan and community resources
  • Familiarity with Social Determinants of Health (SDOH) needs is advantageous
