Chronic Care Management Assistant

Job Summary

A company is looking for a Chronic Care Management-Medical Assistant or Certified Nurse Assistant.

Key Responsibilities

• Develop and maintain patient coaching relationships to support care plans
• Monitor patient adherence to care plans and communicate findings to the care team
• Assist new patients in the enrollment process and act as a liaison between patients and providers

Required Qualifications

• High School diploma or GED
• 1 year of experience in remote chronic care management
• Experience with cardiology patients, particularly heart failure
• Medical Assistant (MA) or Certified Nurse Assistant (CNA) certification
• Familiarity with EMRs, such as Athena, NexGen, GEMMS, and e-clinical works
