Georgia Licensed Care Coordinator

Job Summary

A company is looking for a Care Coordinator to provide care coordination for Medicaid, Medicare Advantage, and Dual Eligible Special Needs Plan members.

Key Responsibilities

• Conduct outreach to educate members about program benefits and facilitate their care
• Assess member status and develop personalized care plans addressing their goals
• Coordinate care with providers and manage a caseload to ensure timely member contact

Required Qualifications

• 2+ years of experience in care coordination, case management, community health work, or related fields
• Experience working with Medicaid and/or Dual Eligible patients
• Remote work experience preferred
• Experience documenting case notes in care management or electronic health record platforms
• Dedicated home office for remote work
