Job Summary

A company is looking for a Social Worker-MSW Senior.

Key Responsibilities

• Assess patients' psychosocial risk factors and provide counseling regarding emotional, social, and financial consequences of illness
• Facilitate engagement and follow-up care, including mental health assessments and crisis intervention
• Serve as a liaison with protective teams and provide intervention in cases of abuse, neglect, and complex discharge planning

Required Qualifications, Training, and Education

• Master's Degree in Social Work required
• De-escalation certification required within 6 months
• Permanent LSW licensure required; LCSW licensure preferred
• 3-5 years of experience in relevant fields preferred
• Proficiency in Microsoft Excel and Word required
