Job Summary

A company is looking for a Care Manager.

Key Responsibilities

• Accountable for Care Management activities for assigned high and moderate members with behavioral health conditions
• Conducts in-depth health risk assessments and develops personalized care plans
• Oversees care support team and ensures effective service delivery and monitoring of care plans

Required Qualifications and Education

• Master's Degree in Social Work or related field (required)
• Licensed Clinical Social Worker (LCSW) or Registered Nurse (RN) certification (required)
• 5+ years of experience in utilization management for mental health and substance health
• 5+ years of care management experience
• Proficient in Microsoft Office and typing skills
