Licensed Clinical Pharmacist

Job Summary

A company is looking for a Clinical Pharmacist Patient Care - Remote.

Key Responsibilities:

• Conduct medication consults with members telephonically to meet/exceed quality metrics
• Identify and address medication adherence barriers while collaborating with patient providers
• Participate in team engagements and support patients in therapy initiation and maintenance

Required Qualifications:

• Current unrestricted and active pharmacist license in the state of primary residence
• Graduation from an ACPE-accredited pharmacy program and passing the NAPLEX
• 1+ years of Clinical Pharmacy Practice experience
• Proven ability to type and talk simultaneously while navigating multiple screens
