Job Summary

A company is looking for an Oncology Social Worker - Outpatient.

Key Responsibilities

• Determine patient and family needs related to social supports and coordinate multidisciplinary case conferences
• Intervene in crisis situations and attend to the needs of patients and families
• Manage referrals for patients at risk and develop appropriate intervention strategies

Required Qualifications, Training, and Education

• Master of Social Work (MSW) required
• Licensed Master Social Worker (LMSW) required
• Compassionate and patient-focused demeanor
• Exceptional documentation and planning skills
• Experience in a healthcare setting preferred
