Job Summary

A company is looking for a Registered Nurse Care Coach to work remotely and support patients in Medicare's Chronic Care Management Program.

Key Responsibilities:

• Utilize care management software to engage with Medicare patients with chronic conditions through monthly calls
• Build rapport with patients to coach them on self-management strategies and improve health outcomes
• Implement and enhance the Plan of Care by updating patient information and connecting them with community resources

Required Qualifications:

• Current, unrestricted New York nursing license
• Proficiency with electronic health records (EHRs) and web-based applications
• Minimum of 3 years' experience as a Registered Nurse
• Self-directed and able to work independently while meeting performance metrics
• Good with technology and eager to learn new software
