Job Summary

A company is looking for a Senior Care Manager (RN).

Key Responsibilities

• Assess, plan, and implement complex care management activities for members
• Develop and monitor personalized care plans in collaboration with providers and caregivers
• Educate members and their families on healthcare options and services available

Required Qualifications

• Active New York State Registered Nurse License
• Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
• 4 - 6 years of related experience
• Experience with care management and collaboration with healthcare providers
• Knowledge of state and federal regulations related to healthcare services
