Job Summary

A company is looking for a Care Manager (RN).

Key Responsibilities

• Evaluate member needs and recommend care plans to address barriers to care
• Develop and collaborate on ongoing care plans with providers and community resources
• Provide education and resource support to members and their families regarding healthcare options

Required Qualifications

• Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
• 2 - 4 years of related experience
• RN - Registered Nurse - State Licensure and/or Compact State Licensure required
• Must reside in Oregon
