Key Responsibilities
- Track incoming referrals and guide families into care quickly and compassionately
- Communicate with referring providers to build trust and provide updates
- Manage follow-ups and ensure no family slips through the cracks
Required Qualifications
- 2+ years in care coordination, medical administration, or customer success, preferably in pediatrics or behavioral health
- Experience with multiple systems (CRM, EHR, email, etc.)
- Able to work independently and meet daily targets
- Strong focus on follow-through and documentation
