RN Care Manager

Job Summary

A company is looking for a part-time RN Care Manager to support maternal health patients through care coordination and remote monitoring.

Key Responsibilities

• Monitor patient data trends and identify high-risk cases requiring intervention
• Perform clinical assessments and provide direct patient education on maternal health concerns
• Support care coordination by assisting patients with referrals and scheduling follow-ups

Required Qualifications

• Active, unrestricted RN license in the U.S. (multi-state compact license preferred)
• Minimum of 3 years of clinical experience in maternal health, OB/GYN, or case management
• Experience in telehealth or digital health platforms is preferred
• Proficiency with EHRs and care management platforms
• Bilingual (Spanish preferred)
