Job Summary

A company is looking for an RN Case Manager.

Key Responsibilities

• Engage with patients to assess and prioritize their clinical support needs
• Coordinate care with healthcare teams and provide education for successful transitions back to home settings
• Implement interventions to improve health outcomes and assist patients in navigating the healthcare system

Required Qualifications

• Bachelor's Degree in Nursing
• 5 years of experience as an RN or RN Care Manager
• Post Acute Care and Transitions of Care experience strongly preferred
• Unencumbered RN license; compact nursing license preferred
• Accredited Case Manager (ACM) preferred
