RN Clinical Care Specialist

Job Summary

A company is looking for a Clinical Care Specialist.

Key Responsibilities

• Manage and monitor a patient census, coordinating Chronic Care Management services
• Create individualized Care Plans and track patient progress, including labs and medications
• Conduct care coordination and provide education to patients and their families

Required Qualifications

• Active RN license in applicable state with compact license
• Five years of nursing experience required
• Strong computer skills and reliable internet access
• Experience with chronically ill patients and telephonic case management preferred
• Self-motivated and able to work independently
