Job Summary

A company is looking for a Manager, Community Health ED Navigator.

Key Responsibilities

• Manage a team focused on transitions of care for emergency room discharges
• Provide clinical oversight and ensure adherence to policies and programming requirements
• Evaluate team performance and participate in quality improvement efforts

Required Qualifications

• At least 3 years of experience as a Registered Nurse in a high-volume acute care setting
• Bachelor's of Nursing from an accredited university
• One year of experience with transitions of care or preventing inpatient/ED readmissions
• Compact licensed RN in TX or FL
• Maintenance of professional credentialing and CME standards
