Job Summary

A company is looking for a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN).

Key Responsibilities

• Administer skilled nursing care to patients in their residence as prescribed by the physician
• Assist in identifying patients' physical, psycho-social, and environmental needs through observations and documentation
• Coordinate care with the interdisciplinary team and document outcomes of interventions

Required Qualifications

• Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing
• Current LPN/LVN license in the practicing state
• Valid driver's license and auto liability insurance
• Current CPR certification
• Proof of Hepatitis consent/declination and TB vaccination record
