Job Summary

A company is looking for an RN Ambulatory Case Manager - Remote PST.

Key Responsibilities:

• Prioritize patient care needs and address emerging issues during initial visits
• Coordinate care with clinicians and staff, ensuring the successful implementation of care plans
• Monitor and coach patients to maximize self-management and continuity of care

Required Qualifications:

• Graduation from an accredited school of nursing
• Active, unrestricted Registered Nurse license through the State of California
• 1+ years of experience in a clinical setting or case management
