Job Summary

A company is looking for a Medicare Diabetes Health Educator, CDCES, Remote.

Key Responsibilities

• Develop personalized education plans for members with diabetes
• Provide ongoing coaching and support for medication adherence and lifestyle changes
• Guide members in accessing community resources and promote health education

Required Qualifications

• 3+ years of related work experience
• Related Bachelor's degree or equivalent experience
• Active NP, RPh, RN, or RD license
• Active Diabetic Educator CDCES license
• Experience working with chronic populations in health education and care coordination
