Key Responsibilities

• Engage members in care coaching and optimize opt-in rates for care programs
• Utilize coaching frameworks to set and refine SMART goals with members
• Monitor member health data and coordinate care within the digital network and external parties

Required Qualifications

• 1-2+ years of experience in health coaching
• Health coaching certification (NBC-HWC or ICF), or personal training certification
• Must be located in specified states
• Experience achieving KPIs related to patient outcomes
• Proficiency in navigating new technologies and systems
