Idaho Licensed Case Management Nurse

Job Summary

A company is looking for a Case Management Nurse with a Behavioral Health Focus (Remote - ID Only).

Key Responsibilities

• Conduct patient assessments and develop individualized care plans
• Coordinate care and facilitate communication among patients, providers, and caregivers
• Perform clinical management, advocacy, and documentation in accordance with program standards

Required Qualifications, Training, and Education

• Minimum of 5 years of clinical nursing experience
• At least 1 year of experience in mental health/substance use nursing and case management
• Associate's or Bachelor's degree in Nursing
• Active and unrestricted Registered Nurse (RN) license in good standing
• Willingness to obtain a Certified Case Manager (CCM) within 5 years of hire
