Job Summary

A company is looking for a Clinical Pharmacist in Kentucky.

Key Responsibilities

• Execute pharmacist-led disease state management via collaborative practice agreement
• Maintain a patient panel for chronic condition disease management including diabetes, HTN, CHF, and COPD
• Collaborate with providers/team to provide exceptional care to patients

Required Qualifications, Training, and Education

• Must hold an active, unrestricted pharmacist license in Kentucky
• At least 3 years of pharmacy experience or ASHP-accredited PGY1 Residency
• Experience in ambulatory care, managed care, or other clinical settings
• Bachelor's degree or PharmD required
• Open to becoming licensed in additional states as needed
