Licensed Care Manager

Job Summary

A company is looking for a Care Manager Transition of Care.

Key Responsibilities

• Evaluate the needs of members by completing post-discharge assessments and medication reconciliations
• Develop care/service plans and collaborate with various healthcare teams to support member transitions
• Facilitate education and resources for members and coordinate services with healthcare providers

Required Qualifications

• Master's degree in Behavioral Health or Social Work, or a degree from an accredited school of nursing
• 2 - 4 years of related experience in care management
• Licensure as LISW, LCSW, LMSW, LMFT, LMHC, LPC, or RN is required
• Ability to work weekends, preferably in the CST time zone
• Experience with telephonic, digital, and home outreach to assess member needs
