Job Summary

A company is looking for a Primary Care Nurse (RN or LPN).

Key Responsibilities

• Manage triage calls and messages to support patient care coordination
• Collaborate closely with providers as part of a care team
• Transition to a hybrid work schedule after completing on-site training

Required Qualifications, Training, and Education

• For LPN: Diploma in Practical Nursing and active LPN license
• For RN: Associate of Applied Science in Nursing or BSN, or 4 years of equivalent experience, and active RN license
• 2-3 years of experience preferred for both RN and LPN roles
