Job Summary

A company is looking for a Behavioral Health Case Manager, LCSW, RN or CPC.

Key Responsibilities

• Perform member assessments across various domains using evidence-based criteria
• Develop and implement individualized care plans, collaborating with members and healthcare providers
• Provide advocacy, health education, and support for members' behavioral health services

Required Qualifications

• Current, unrestricted LCSW, RN, LMFT or CPC license
• Nevada state license
• 2+ years of behavioral health experience
• 2+ years of case management experience
• Intermediate proficiency in Microsoft Word and Windows environment
