New Jersey RN Case Manager

Job Summary

A company is looking for an RN Case Manager (Care Connect) - 100% Remote.

Key Responsibilities:

• Assess patients' clinical needs to ensure appropriate care levels and lengths of stay
• Coordinate and implement care plans while monitoring the effectiveness of services provided
• Serve as a mentor to new RNs and act as a subject matter expert for various projects

Required Qualifications, Training, and Education:

• High School Diploma/GED required; Bachelor's degree preferred or relevant experience in lieu of degree
• Minimum of two (2) years clinical experience required
• At least two (2) years of healthcare payer experience required
• Active Unrestricted NJ RN License or active Compact License required
• CCM certification preferred
