Job Summary

A company is looking for a Licensed Social Worker for multiple openings in a remote capacity.

Key Responsibilities

• Manage members with chronic or catastrophic illnesses and complex psychosocial conditions
• Coordinate care across various settings, focusing on transitions and member education
• Assess members' needs and facilitate access to appropriate services and supports

Required Qualifications, Training, and Education

• Bachelor's degree required
• Master's in social work (LMSW) required
• Certification in clinical social work (LCSW) preferred
• 4 - 6+ years of case management and/or managed care experience
• Knowledge of Federal and State regulations for Medicare and Medicaid
