Nurse Practitioner Partner

Job Summary

A company is looking for a Nurse Practitioner Partner (Multiple States).

Key Responsibilities

• Conduct 5-10 hours per week of telehealth visits, including consultations and medication prescriptions
• Collaborate with Community Health Workers (CHWs) to assess patient needs and eligibility for services
• Provide clinical oversight to CHWs and maintain accurate patient records

Required Qualifications and Experience

• Board certification as a Nurse Practitioner
• Minimum of 2 years of consecutive experience as a Nurse Practitioner
• Commitment to health equity and addressing social determinants of health
• Ability to operate independently with strong diagnostic skills
• Experience in telemedicine and proficiency with various digital tools
